Evaluation and Statement of Credit System

Thank you for attending the ADHA 2021 Annual Conference, welcome to the evaluation site.

If you attended both the in-person version of the conference, and virtually, you cannot claim credit for duplicate sessions. This includes Opening & Closing General Sessions, Exhibit Hall and Education Sessions.

Attendees who attended sessions live in Phoenix will only be able to evaluate 1 session per timeslot. This rule does not apply to virtual conference attendees.
